About the Role
This vital role is an opportunity to make a real difference in the lives of children, young people, and families. As a Support Practitioner, you will be providing high-quality support to ensure their safety and well-being.
Key Responsibilities:
* Facilitate intake, screening, and placement of children and young people with foster and kinship care families by working closely with carers, parents, and community partners.
* Deliver assessments, crisis support, and therapeutic interventions to meet individual needs, escalating concerns as necessary for better outcomes.
* Develop and evaluate care plans tailored to each child or young person's unique circumstances, collaborating with professionals from other agencies when needed.
* Implement evidence-based parenting programs, working with local communities to support families in need and promote positive relationships.
* Recruit, assess, and support foster and kinship carers through thorough approval processes that prioritize their well-being and capacity to provide quality care.
* Lead case management planning and maintain accurate client records to inform decision-making and ensure seamless services.
* Provide flexible, in-home support services including after-hours and on-call duties to meet diverse family needs.
* Work respectfully with Aboriginal and Torres Strait Islander communities to ensure culturally safe and inclusive practices that value their experiences and perspectives.
Requirements:
* A degree in social work, psychology, or a related field, or equivalent experience, allowing you to apply theoretical knowledge in practice.
* A strong understanding of child protection legislation and policies to safeguard vulnerable children and young people.
* Expertise in conducting thorough care assessments and trauma-informed practice to support complex family situations.
* Knowledge of the challenges faced by Aboriginal and Torres Strait Islander communities, enabling culturally sensitive approaches to service delivery.
* A current driver's licence, National Police Check, Working with Children (Blue Card), and Licensed Care Service (Child Protection) are essential requirements for this role.
