What You'll Do:
In this dynamic and rewarding role, you'll:
* Partner with participants to design and implementpersonalised, strengths-based NDIS plans.
* Connect people with the services and supports that alignwith their goals and needs.
* Use recovery-oriented and person-centred approaches tobuild skills and confidence.
* Coordinate service delivery, monitor progress, and adaptplans as required.
* Collaborate with a multi-disciplinary team and contributeto service innovation.
* Maintain accurate records and ensure alignment with NDISpractice standards.

What You'll Bring:
* Significant experience in the disability or human servicessector.
* Knowledge of intervention models and ability to coordinateservices.
* Specialist knowledge of NDIS and related supportservices.
* Strong problem-solving and conflict-resolution skills.
* Ability to advocate effectively for participants andpromote their independence.
Your Toolkit:
* Tertiary qualification in community services, social workor equivalent and extensive community work and training(essential)
* Current SA driver's licence (essential)
* Current Safe Environments for Children and Young People– Through Their Eyes Certificate (essential)
* Current DHS Working with Children Check (essential)
