Brisbane
Trilogy Care Pty Ltd
Posted: 18 March
Offer description

Trilogy Care is one of Australia's fastest-growing approved Support at Home package provider. Our innovative self‐management model gives older Australians more choice, transparency, and value - so they can spend less on fees and more on the care they need.

We're a vibrant, purpose‐driven team dedicated to helping people live independently at home for longer. It's not just about compliance - it's about making a real difference for our clients, their families, and our people. We're proud to be building a workplace where individuals feel empowered, supported, and valued.

About the Role

The Clinician (Clinical) supports Trilogy Care's clinical governance framework, helping ensure safe, high‐quality and compliant care across the organisation. This is a system‐focused role centred on governance, clinical risk oversight and continuous improvement rather than direct service delivery.

You will triage and oversee clinical incidents, support compliance with the Serious Incident Response Scheme (SIRS), and monitor clinical data to identify trends and emerging risks. The role contributes to incident reviews, root cause analysis and quality improvement initiatives that strengthen care quality and organisational learning.

Working closely with the Head of Clinical Governance, operational leaders and care teams, you will provide specialist clinical advice and support strong governance processes, regulatory compliance and improved safety outcomes across the organisation.

What you'll do
* Triage and oversee all organisational incidents, ensuring appropriate prioritisation, escalation and timely resolution in line with regulatory and organisational requirements.
* Provide expert clinical advice to leaders and care teams on incident management, clinical risk and safe care responses.
* Monitor incident data, SIRS notifications and clinical risk trends to identify emerging issues, systemic risks and opportunities for preventative action.
* Support root cause analysis, case reviews and incident investigations to strengthen organisational learning and improve care quality.
* Ensure incidents, actions and outcomes are accurately documented and corrective actions are implemented and monitored for effectiveness.
* Support the implementation and ongoing improvement of clinical governance frameworks, ensuring compliance with the Aged Care Act, Strengthened Aged Care Quality Standards and Support at Home obligations.
* Analyse clinical indicators and governance data to inform reporting, identify trends and support continuous quality improvement initiatives.
* Work collaboratively with internal teams and external stakeholders to strengthen governance processes, build capability and support safe, high‐quality care outcomes.
What you'll bring
* Tertiary qualification in a relevant healthcare discipline and current unrestricted AHPRA registration with the appropriate governing body.
* Minimum 3 years' clinical experience, ideally within aged care, home care or community health.
* Strong clinical assessment skills, critical thinking and sound professional judgement.
* Solid understanding of incident management, clinical risk and regulatory compliance requirements.
* Excellent written and verbal communication skills, with the ability to prepare clear documentation and governance reporting.
* Ability to collaborate across teams and provide practical, solution‐focused clinical advice to stakeholders.
* Experience in home care, community care or aged care service environments.
* Experience in incident investigation, clinical governance, risk mitigation or quality improvement activities.
* Knowledge of the Support at Home Program, aged care legislation, quality standards and SIRS obligations.
