DH Caring Heart is a community-based disability support provider delivering personalised care, in home assistance and community participation programs across Victoria. We are seeking a dedicated Disability /Community Support Worker to join our team and provide high quality support to clients with diverse and sometimes complex needs.
About the Role
The successful applicant will work directly with clients to enhance independence, wellbeing and social participation. This position involves:
· Meeting with clients to understand their support needs, daily living requirements and personal goals
· Developing and delivering tailored support plans, independent living skill programs and community access activities
· Monitoring client progress, completing accurate case notes and reporting changes in behaviour, health or risk indicators
· Referring clients to relevant external health, welfare or community services when further assistance is required
· Liaising with local service providers, community organisations and government bodies to coordinate care
· Supporting families and carers with guidance, education and respite assistance
Providing safe personal care, routine assistance and support in home and community environments
· Assisting young people and adults experiencing social, emotional or financial challenges
· Preparing internal reports, risk assessments and information for service improvement initiatives

Pay: $24.95 – $50.68 per hour
Expected hours: 38 per week
