Greenway Views in Tuggeranong, Canberra, is home to nearly 400 residents across 327 apartments with premium on-site facilities including cafés, theatre, gym, library, chapel, art centre, salon, and dog park.About the RoleReporting to the Clinical Case Manager Lead, the Clinical Case Manager supports integrated and coordinated resident care. 
Responsibilities include conducting pre-entry clinical assessments, providing move-in support, developing and reviewing care plans, overseeing care delivery and evaluation, and coordinating referrals for externally funded care (including all associated documentation). 
The role focuses on ensuring residents receive the right support at the right time through effective case coordination, service planning, and networking with internal and external stakeholders. 
No two days are the same, offering exposure to residents and their families while building strong, meaningful connections.
