Eramosa Family Medical Centre was established in 1992 with the desire to provide a broad range of health services to families on the Mornington Peninsula, based on a strong foundation of care and respect. We are a newly renovated, busy clinic with 20 General Practitioners and a team of 5 experienced GP nurses
We are looking for a dynamic, enthusiastic and professional Div 1 or 2 nurse who has the ability to work as a general practice nurse with our team of valued GP nurses.
This is a permanent position and a fantastic opportunity to show initiative and join a team of dedicated health professionals. We are looking for a nurse who is able to work:
Tuesdays 9-5.30pm, Wednesdays 11.30-5.30pm and Thursdays 9-5.30pm, and 1 in 4 Saturdays 9-1pm (plus help with covering shifts when others are on leave)
Essential
* Current AHPRA registration
* Pleasant and professional manner with a commitment to providing excellent patient care
* Ability to work independently and in a team environment
* Efficient time management skills
* Sound computer knowledge (Best Practice experience highly regarded)
* Training provided for the right candidate
Desirable
* General practice experience
* Experience in Accreditation process
* Chronic disease management
* Wound management experience
* Accredited Nurse Immuniser
* Ability to work well under pressure and to multi-task in a busy environment
Responsibilities will include:
* General practice nursing duties including triage, wound care, chronic disease management, assisting with procedures, immunisation, and health assessments.
* Managing the clinical environment
* Responsibility for the Cold Chain Management and infection control processes
* Maintaining stock of clinical supplies
* Provide care for patients with chronic and complex care needs.
* Perform detailed Health Assessments and Care Plans for patients, and initiate Home Medicine reviews in collaboration with the GP.
* Support/Educate clients to achieve self-management of their chronic disease(s).
* Maintain a recall and reminder system
* Maintain patient My Health records and register patients for My Medicare
* Attend meetings/educational sessions
