As a Clinical Care Assessor at CatholicCare, you’ll step into a flexible, community-based role where you’ll work directly with older Australians to understand their complex care needs and help them to access the right care and support.

What You’ll Do:
1. Conduct face-to-face assessments with older people in their homes with no hands-on care requirements.
2. Develop person-centred care plans that support independence and wellbeing.
3. Collaborate with a multi-disciplinary team of nurses, allied health professionals and care partners.
4. Help clients and families navigate complex aged care services with compassion, clarity and confidence.
What You’ll Bring:
1. Tertiary qualifications as a Registered Nurse, OT, Physio or Social Worker with current AHPRA registration (or equivalent).
2. A desire to transition into community-based work, bringing your clinical skills into a more person-centred setting.
3. Great communication and collaboration skills, with the ability to build trust with clients and work as part of a supportive team.
4. A C Class Driver’s Licence
5. Comfort with digital tools and systems (training will be provided).
If you’ve completed My Aged Care Assessor training or have experience in aged care, disability, or health assessments that’s a bonus, but not essential.
Flexible Hours to Suit You:
We’re hiring 3 new Clinical Care Assessors in Rockhampton with part-time or full-time options available. There is flexibility to scale your hours as service demand grows over time. Start with two days a week or more if you’re ready.
