About The Role
Working with GPs, psychiatrists, and other health professionals, the Clinical Care Planner will coordinate care for people with severe and complex mental illness. Using a recovery-oriented framework, the role involves developing joint care plans with patients and their support networks to improve health outcomes, support effective symptom management, and reduce unnecessary hospitalisation. Key responsibilities include care planning, supporting stepped-care approaches in primary care, and liaising with health professionals to ensure coordinated and effective care.
Experience Requirements
* Demonstrated experience working within a fast-paced environment.
* Proven successful delivery of Clinical Care and Coordination services or other relevant mental health services.
* Experience in working with adults with severe and complex mental health needs
* A minimum 2 years supervised experience in the area of mental health.
* A minimum 2 years relevant professional development and or supervision.
* Experience in delivery of evidence based mental health interventions and assessments.
Qualification And Other Requirements
* Knowledge and experience in using the Microsoft suite of software and databases.
* Mental Health Nurse, Mental Health Accredited Social Worker, Mental Health Accredited Occupational Therapists or Psychologists
* Current and full registration with the appropriate registration authority
* Current or willing to obtain a Working with Children\'s Check (WWCC)
* Current or willing to obtain a National Police Clearance
* Current Child Safe Environments
* Driver\'s Licence
