We are seeking a skilled and compassionate Registered Nurse to fill this Care Coordinator position.

Key Responsibilities:
* Conduct thorough assessments and care reviews to ensure the highest level of service delivery.
* Develop and review personalized care plans, taking into account client needs and funding requirements.
* Provide training and guidance to Support Workers to maintain their clinical competencies.
* Collaborate with other teams to deliver coordinated and holistic care.
* BUILD STRONG RELATIONSHIPS WITH CLIENTS, FAMILIES, AND STAKEHOLDERS TO ENSURE EXCELLENT CARE EXPERIENCES.
Requirements:
* Current AHPRA registration as a Registered Nurse.
* Proven experience as a RN, ideally within aged care, disability, community, or primary care settings.
* Strong clinical assessment, care planning, and training skills.
* Ability to oversee and maintain Support Worker clinical competencies.
* EXCELLENT COMMUNICATION AND INTERPERSONAL SKILLS TO BUILD TRUST WITH CLIENTS AND TEAM MEMBERS.
