What You'll Do
Provide clinical nursing care in the home — wound care, catheter and stoma management, diabetes support, and chronic disease management.
Complete health assessments and develop personalised care plans.
Collaborate with multidisciplinary crew - GPs, carers, and allied health professionals, care coordinators for truly holistic support.
Educate clients and families, giving them confidence to manage their health.
What We're Looking For
Division One RN, registered with APHRA & minimum 1 year of experience in community, aged care, or acute settings
Proficient use of technology, including mobile platforms and provision of telehealth services.
Police check, Working with Children check, CPR, Valid driver's licence and access to a reliable, insured vehicle
Passion for delivering person-centred care in the community
Willingness to travel for client care for up to 2 hours (Paid travel time)
