About the Team The BCH Community Nursing Program provides high standard nursing care to people in their own homes and other community settings, enabling them to maintain quality of life. We work in conjunction with Palliative Care Community Nurses and Occupational Therapists with access to Specialist Medical support. About the role The Community Nurse will provide holistic, value-based health care, in a collaborative manner, enabling and empowering people to remain in their place of choice. This position is responsible to facilitate a high-quality service provision that is responsive to clients’ need and empowers them to self-manage where appropriate. To be successful in this role, you will have: Current Australian Health Practitioner Regulation Agency (AHPRA) registration as a Registered Nurse. At least 3 years’ nursing experience, demonstrating commitment to client-centred care. A compassionate and professional approach to client care. Are able to work autonomously, while retaining the ability to work as a collaborative team member. Excellent communication, organisational and interpersonal skills. Strong clinical decision making and problem-solving skills. An ability to provide professional, accurate and evidence-based nursing care and advice to all stakeholders.
