About the role
Support individuals transitioning from Murrumbidgee Local Health District inpatient and community mental health, drug and alcohol teams. Ensure continuity of care, coordinate access to services, and empower clients in recovery-focused pathways. Collaborate with hospital teams, community services, families, and stakeholders for safe discharge planning and post-discharge support. Prioritise Aboriginal, CALD, and other target populations. Cover Wagga and Young LGAs. Full-time with Rostered Day Off option.
This is a full-time position, fixed term until January 2027 with the option for a Rostered Day Off each month. The role will cover Wagga and Young LGAs.

Duties
• Support transition and discharge planning for mental health clients
• Coordinate access to appropriate community services
• Empower clients to engage in recovery-focused pathways
• Collaborate with hospital, community, and family stakeholders
• Prioritise support for Aboriginal, CALD, and other target populations
Skills & Experience
• Current full AHPRA registration as a nurse, clinical psychologist, registered psychologist, or occupational therapist (OTA accredited in mental health), or Social Worker (min. 2 years post-grad mental health experience, AASW eligible)
• Demonstrated clinical experience in mental health service delivery, providing holistic, recovery-oriented, evidence-based therapeutic services
• Skills in professional supervision, field supervision, coaching, and mentoring
• Experience coordinating and facilitating student placements
