About The Role
* You will work within a small team, providing community-based approach to the Barkly remote communities and outstations in accordance with our Primary Health Care Delivery Model.
This is an exciting opportunity for you to join us on this journey, addressing health care needs for Aboriginal peoples and their families residing in our four remote Barkly communities/outstation service delivery areas (Wogyala, Corella Creek, Mungkarta and Nuramin including; other smaller Barkly outstations that may have Anyinginyi clients and families residing from time to time).
Key Responsibilities
1. Plan and provide regular remote outreach community screening, health assessments and individual or family's needs per CARPA standard treatment manual.
2. Tailor your approach to address specific healthcare concerns unique to each family while building effective relationships.
3. Collaborate with AHAC clinics and CDC staffs for improved continuity of patient care. Ensure thorough knowledge of all relevant systems such as Northern Territory CARPA manuals prior consultations begin.
