About you:

* Minimum 1 year experience in community or social support
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Diploma of Community Services (or equivalent)
* A compassionate, reliable, and proactive attitude
* Ability to build positive relationships with clients and stakeholders
* A commitment to promoting inclusion and independence
Tasks & responsibilities:
* Advocate for clients' rights, access, and inclusion within the community
* Provide person-centred support tailored to individual needs
* Develop and implement individualised support plans
* Support clients with community engagement and participation
* Promote independence, wellbeing, and life skills
* Refer clients to appropriate specialist services and agencies
* Maintain accurate documentation and progress notes
* Work collaboratively with team members and external providers

Experience:
* Community support worker: 1 year (Preferred)
Licence/Certification:
* Diploma of Community Service or Cert III (Preferred)
