Disability Support Coordinator Role Summary

This role involves working with individuals with disabilities to develop and implement support plans that promote their independence, autonomy, and participation in the community.

As a Disability Support Coordinator, you will work closely with NDIS participants, their families, and other community supports to ensure they receive the support they need to achieve their goals.

">

The ideal candidate will possess strong communication skills, empathy, and the ability to build rapport with clients from diverse backgrounds.

">

You will also have experience working in a paid or voluntary capacity with people with disabilities and be familiar with the principles of person-centered planning.

">

Additionally, you will hold a current Australian driver's license and Working with Children Check, and have relevant qualifications in a human services field.

Key Responsibilities:

* Develop and implement support plans for NDIS participants
* Work collaboratively with participants, families, and community supports to achieve participant goals
* Provide advocacy and support to help participants navigate the NDIS system
* Conduct regular reviews and updates of participant support plans
* Maintain accurate records and reports as required

Required Skills and Qualifications:

* Relevant qualifications in a human services field (e.g., Community Services, Case Management, Disability, Mental Health, Youth Work)
* Experience working in a paid or voluntary capacity with people with disabilities
* Strong communication and interpersonal skills
* Empathy and ability to build rapport with clients from diverse backgrounds
* Current Australian driver's license and Working with Children Check
