The Safer Care Nurse plays a vital role in delivering high-quality healthcare services to individuals with complex needs. This position requires the nurse to work independently and as part of a multidisciplinary team, providing care and support to participants accessing safer pharmaceutical alternatives.
About the Role:
* Assess and screen participants for health concerns, substance use, mental health, and other related issues.
* Develop and maintain individualized care plans, providing ongoing monitoring and support.
* Collaborate with community partners to ensure seamless support for mutual participants.
* Administer medications as ordered by program physicians and according to clinical protocols.
Key Qualifications:
* Registered Nurse (RN), Registered Psychiatric Nurse (RPN), or Licensed Practical Nurse (LPN) registered with the relevant regulatory body.
* Minimum 1-5 years' full scope nursing experience working with individuals with poly-substance use and mental health challenges.
* Previous experience working in supervised consumption/overdose prevention services, safer supply, or other harm reduction programs.
