About the Role
This is an exciting opportunity to join our multidisciplinary team as we provide assessment, care coordination, intervention, and follow-up services.
Key Responsibilities
* Patient Assessment and Care Planning: Conduct comprehensive assessments to identify patient needs and develop individualized care plans.
* Care Coordination: Collaborate with consumers, their families/carers, GPs, Community Managed Organisations (CMOs), and other service providers to ensure safe and effective care.
* Eligibility Criteria: Be eligible to drive in NSW and interstate, and willing to travel for work-related purposes.
* Skills and Qualifications: Possess knowledge and experience in mental health assessment, care planning, and contemporary practice.
* Effective Communication: Communicate effectively with consumers, families/carers, and healthcare professionals.
