About The Role
Providing expert advice and active engagement with Northern NSW Local Health District (NNSWLHD) facilities and services to facilitate improved practice and sustainable change in the prevention and management of falls, and the prevention (where applicable), identification and management of cognitive impairment.
Assisting in the provision of comprehensive care in sites and services by ensuring NNSWLHD has the required governance, systems and processes in place to effectively plan, manage and coordinate comprehensive care planning and shared decision making. Working collaboratively with all relevant comprehensive care committees, clinicians and patients/carers to develop, coordinate and support the implementation of a multifaceted, evidence based and sustainable approaches to health care, regardless of the setting.
