BlueCare has embarked on an exciting transformation to strengthen the provision of quality care in the community setting. Being part of this established centralised function of Statewide Dementia Care team, you will work with a team of passionate, like‐minded professionals. With a dedicated focus on providing high‐level care based on assessed client needs, you will promote quality of life and help individuals maintain their independence and remain in their homes.
Your part in our Statewide Specialised Team
Reporting to the Dementia Care Manager, Statewide Specialised Service, you will support BlueCare's neighbourhoods by providing support, guidance and strategies to personal carers, ensuring high‐quality in‐home dementia care.
Key Responsibilities
* Deliver direct in‐home or telehealth support to people living with dementia and their families.
* Provide information and tools tailored to individual circumstances.
* Assist with access to BlueCare support services and programs to maintain health and wellbeing.
* Deliver carer support.
* Teach techniques to cope with individual behavioural concerns.
* Advise on meaningful activities and equipment from a comfort interventions library to engage people living with dementia.
* Under the guidance of the Dementia Consultant, carry out care planning and assessment as per review schedule and changes in clients' needs.

Qualifications & Experience
* Relevant qualifications and/or demonstrated experience in community‐based or health‐related services, especially in dementia. Cert IV or higher qualification considered.
* Experience working with carers of people with dementia, people living with dementia and their families; ability to recommend/offer improvements to their quality of life.
* High level of verbal, written and interpersonal communication.
* Ability to network with, consult and respond to diverse stakeholders.
