Where you'll be working
This is a registered nursing position which is based at Auburn Community Health Centre and is required to provide care clients in their homes and community nursing clinics within the Western Sydney Local Health District. This position is required to work occasional afternoon shifts, weekends and public holidays. These shifts are worked out of the Blacktown Community Health Centre. The current requirement, (that is always subject to change), is approximately 1-2 afternoon shifts and 1-2 weekend shifts per 4-week roster period.
What You'll Be Doing
In this role, you will provide comprehensive nursing care to clients with chronic and complex health needs, working both independently and collaboratively to support optimal health outcomes.
* Conduct holistic nursing assessments and develop, implement and evaluate care plans
* Coordinate care and deliver evidence-based clinical interventions in the community
* Educate and support clients, carers and families to promote self-management
* Maintain accurate, timely clinical documentation and ensure safe handover practices
* Contribute to continuous improvement, clinical reviews and professional development

* Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)
