Overview
Senior Case Manager. Permanent Full Time - Dandenong at Life Without Barriers. This role provides intensive case management support to individuals and families with complex needs within the Status Resolution Support Services program, ensuring high-quality, evidence-based, and culturally responsive services through needs assessments, case planning, referrals, and ongoing review.
Based in our Dandenong Office, with occasional work from home. This role offers a monthly rostered day off (RDO) and generous employee benefits including salary packaging and Fitness Passport.
Responsibilities
* Provide intensive case management to individuals and families with complex needs within the Status Resolution Support Services program.
* Ensure delivery of high-quality, evidence-based, culturally responsive services through needs assessments, case planning, referrals, and ongoing review.
* Provide guidance, mentoring, and training to Case Coordinators and Cultural Support Workers.
* Contribute to service improvement, compliance, and collaboration with internal and external partners to achieve positive outcomes for clients.
Qualifications
* A recognised Degree qualification in Social Work, social sciences, psychology, nursing or mental health or a recognised diploma in a related field and working toward a degree qualification.
* Proven experience in case management, working with individuals presenting with complex needs including trauma, mental health, physical health, child protection and family violence.
* Ability to work autonomously while effectively managing competing priorities in a fast-paced environment.
* Skilled in applying trauma-informed, strengths-based and culturally responsive practice to achieve positive outcomes.
* Demonstrated leadership capability, including mentoring, coaching, training and supporting staff.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and any relevant state-based checks.
