ABOUT THE ROLE
We are looking for exceptional Care Coordinators who are passionate health/aged care professionals looking to join our Caloundra, Caboolture & Gympie offices on a full-time basis. As Care Coordinator you will be an integral point of contact for workers and clients to provide support for the broader community team.
As a Care Coordinator you will be responsible for:
o Building strong and meaningful connections with clients and their family
o Conducting comprehensive client assessments and development of care plans to support our clients to remain within their home
o Interacting and providing professional health and clinical care advice and direction to a team of home care workers
o Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction
o Ensuring all practices are consistent within policy and legislative frameworks
o Maintaining up to date client records and information
To be successful you must hold:
o Formal tertiary qualification eg. Bachelor of Nursing or equivalent
o Current AHPRA registration as an Enrolled or Registered Nurse
o Demonstrated knowledge within the Aged Care sector
o National criminal history/police check
o Industry applicable licences (eg. AHPRA registration, BC/NDIS)
o Valid First Aid and CPR certificate
o Minimum of 2 COVID vaccinations
o Open drivers licence and reliable transportation
