About the Way Back Program:
The Way Back (TWB) Program was first developed by Beyond Blue in 2014, a psychosocial service response to provide support for people following a suicide attempt or experiencing a suicide crisis. The program is now delivered nationally across most states and territories in Australia.

The opportunity:
We're excited to offer a full-time Care Coordinator position based primarily in Gympie, working across beautiful Gubbi Gubbi country. In this role, you'll provide individualised, practical psychosocial support to individuals and their families/carers referred to the service by the Hospital and Health Services after a suicide attempt or during a suicidal crisis. This is a max-term contract until 30 June 2026, based out of our Gympie office with team connections to Maroochydore.

This service is all about building connection, maintaining regular and meaningful contact, and helping individuals access both formal and informal supports. You'll work closely with people to identify and address the barriers contributing to distress and support them in planning for life beyond the 12-week program.

Responsibilities include:

* Provide non-clinical care and practical psychosocial support for individuals and their families referred to the service for the first 3 months following a suicide attempt or crisis.
* Engage with individuals referred to The Way Back, developing trust and supporting them in a culturally appropriate, trauma-informed, and professional manner.
* Work collaboratively with individuals to develop, implement, and review support plans, including a comprehensive assessment of needs and safety planning.
* Build and maintain strong relationships with the individual, their supports, service providers, hospitals, GPs, and health services.
* Participate in the collection and evaluation of data to monitor and report on program effectiveness.

About you:
You are passionate about engaging with people in distress, and you bring a calm, professional, and trauma-informed approach to your work. We are looking for someone who is warm, responsive, and team-oriented, with the following skills and experience:

* Tertiary qualifications in Mental Health or Social Services-related fields.
* Experience in mental health services.
* A committed, collaborative approach to supporting people with complex needs.
* Ability to work independently while remaining connected to a supportive team.
* Excellent organisation and communication skills.
