Position Summary
The MCCN provides care, information, and support for people with cancer across care pathways and multidisciplinary teams within the cancer care community.
The MCCN offers collaborative clinical and supportive care, leadership to develop clinical practice aligned with the McGrath Model of Care, and expert clinical advice to patients, carers, and health professionals with complex needs.
Responsibilities
Provide leadership and expert clinical guidance across cancer care, including health assessment, treatment pathways, referrals, and ongoing care facilitation.
Develop, implement, and evaluate care management plans for patients diagnosed with specific cancers, focusing on complex care coordination.
Develop education and training programs; participate in collaborative research related to Cancer Care Services.
Offer client- and family-centered care through the continuum of care from diagnosis to discharge or terminal care; coordinate communications among multidisciplinary teams and with General Practitioners and community health services.
Schedule treatments, triage patient needs, and coordinate referrals to allied health and support services; advise on complex discharge planning.
Contribute to treatment planning within the multidisciplinary team and support data entry for McGrath Foundation database; participate in McGrath Foundation education events.
Qualifications & Requirements
Applicable vaccination requirements for a Category A role
Working With Children Check (WWCC)
Current AHPRA Registration
Minimum five years full-time equivalent post-registration experience, with at least three years in Oncology, Palliative Care, or Cancer Stream nursing; post-graduate qualifications relevant to the field
Valid NSW Driver's Licence
This is a temporary funded position until 27 June 2027 with the possibility of extension subject to funding.
