As a Lifestyle Coordinator at Avature, you will have the chance to make a real difference in the lives of our residents. You will work closely with our General Manager and care team to develop and manage programs and events that cater to the diverse interests and needs of our residents.
Your Responsibilities:
* Lifestyle Programs: Develop individualized lifestyle programs based on resident needs, preferences, and goals to enhance physical, intellectual, emotional, cultural, and social well-being.
* Active Participation: Lead and participate in activity delivery to ensure they are engaging and meaningful for residents.
* Lifestyle Assessments: Prepare thorough and accurate lifestyle assessments of residents to inform personalized care plans.
* Documentation: Ensure timely and complete documentation of resident participation and engagement.
* Team Management: Manage and coach the lifestyle team to deliver exceptional lifestyle activities.
* Training and Orientation: Support new team members and volunteers through orientation and training.
* Relationships: Develop and maintain strong relationships with residents, families, and volunteers.
* Community Engagement: Facilitate opportunities for residents to engage with the broader community.
About You:
* Leveraging Expertise: Demonstrated understanding of leisure and lifestyle programming, including Aged Care Quality Standards.
* Certified Professional: Certificate IV in Leisure and Health.
* Relevant Experience: Previous experience in a Lifestyle Assistant/Coordinator position (or similar), including dementia care.
* Tech Savvy: Proficient in using computerized care management systems and possessing excellent computer literacy skills.
* Effective Communicator: Strong written and verbal communication and interpersonal skills.
