Desired Skills And Experience
The Opportunity
We're looking for passionate and dedicated Clinical Care Coordinators to join our Youth Enhanced Service (YES) team. In this role, you'll provide mental health care coordination for young people aged 12–25, as well as their families and carers, ensuring they receive the support they need to thrive.

As a Clinical Care Coordinator, you'll work with young people experiencing a range of mental health challenges. You'll be part of a multidisciplinary team committed to delivering evidence-based, developmentally appropriate, and family-inclusive care.

What We 're Offering

* Full time position available based in Toowoomba at the Youth Enhanced Services office

What We're Looking For

* A degree in Psychology, Social Work, or a related discipline (minimum three-year undergraduate in medical or allied health) plus post-graduate experience supporting young people.
* Proven ability to monitor risk, manage complex presentations, and coordinate care for young people with diverse needs.
* Experience working with young people facing mental health challenges, disabilities, alcohol and other drug issues, and social complexities.
* Ability to connect with young people, families, carers, community agencies, and stakeholders to create a supportive network.
* Skilled in helping young people develop independence—budgeting, building social connections, and accessing community resources.
* Confident in engaging groups and tailoring communication to different age ranges and audiences.
* Understanding of the unique experiences of young people from diverse backgrounds, including CALD communities, Aboriginal and Torres Strait Islander peoples, and LGBTQIA+ youth.
* Experience running programs that empower young people and strengthen family and peer support.
* Ability to foster strong partnerships across community and professional networks to improve youth health outcomes.

Desirable

* Knowledge of local youth and health sectors
* Familiarity with the Youth Enhanced Service Model (Orygen)
* Understanding of trauma-informed practice

Other Requirements

* This position requires travel, telehealth, home visits, and overnight stays. Flexibility in hours is necessary for this role.
* Successful applicants will also need to hold a 'C' class Open driver licence, State Working with Children Check.
