Support Professional for Dementia Care

We are seeking an experienced and compassionate individual who is able to share knowledge and expertise in a way that supports change at all levels. Your high level of negotiation, communication and relationship building skills will enable you to work effectively with stakeholders, including families, caregivers and clinical staff.
You will be responsible for supporting families and caregivers to understand how dementia impacts each individual, and equipping them to prevent and respond to behaviour changes through modelling, coaching and development of resources.
* Synthesising information to provide recommendations that are relationship centred and informed by evidence based best practice.
* Developing reports that are contextualised to individual situations and people, that effect change on the ground and improve quality of life for people living with dementia.
* Undertaking a range of assessments that help to understand the holistic experience of the person and their care network.
You will be working in partnership with the broader sector to build capability across home and aged care sectors. We are looking for someone with empathy and patience, who can contribute to research, projects and programs and work innovatively to meet the changing needs of the sector.
The ideal candidate will possess expertise in supporting people living with dementia, experience in Australian residential aged care and or community aged care, and relevant qualification in Allied Health, Nursing, Aged Care or Mental Health. You will be required to travel and hold a current Australian driver's license.
