At the Healthscope Independence Services (HIS), we are seeking a dedicated and proactive Intake and Support Officer to connect participants, families, support workers, and management while supporting smooth and compliant onboarding across our services.

Permanent, PartTime (4 days a week)

Classification: Level4 – Pay point will be determined based on experience and qualifications.

Primary office location - Victorian Rehabilitation Centre, Glen Waverley

About the Role

This role will report to the Intake & Service Implementation Manager. The Intake Officer will play a key role in supporting prospective clients and their families from their first enquiry through to the commencement of services. You will manage incoming referrals, identify client needs, and coordinate appropriate supports ensuring a smooth, professional, and supportive experience at every step. While our support office is based at the Vic Rehab Centre (VRC), this role will require some community-based work, requiring travel for client onboardings, community engagement, networking activities, and participation in expos.

A typical day may include:

Handle general enquiries and provide follow‐up support to participants, families and external stakeholders.

* Work closely with support coordinators, families, clinicians, and other external stakeholders to confirm service capacity and readiness.
* Coordinate participant intake, onboarding and service commencement process.
* Attend new resident training, care team meetings and handover sessions as required.
* Prepare and follow up risk assessments, service agreements, referral documents and individual support plans.
* Work with operational teams and leaders to ensure a smooth intake and positive service delivery experience.
* Represent the organisation professionally at care team meetings, expos and events while building strong relationships with key stakeholders including hospitals, allied health, Support Coordinators and governing bodies.

To be considered for this role you will have:

Cert IV in Disability, Community Services, or Individual Support, or a relevant Diploma/Degree (Social Work, Community Services, Psychology, Human Services).

* Previous experience in intake, assessment or case management within the disability sector or related support services.
* Strong organisational skills, with the ability to manage competing priorities and work autonomously.
* Understanding of participant onboarding, compliance requirements and NDIS / TAC funding.
* Strong customer engagement, communication, and negotiation skills, with a commitment to high quality, tailored service.
