The People Project Tasmania (TPPT) is a not-for-profit organisation and one of seven government-funded Rural Workforce Agencies (RWAs) nationally. We support Tasmanian communities by strengthening the health workforce and delivering support coordination, plan management and aged care coordination services across the state. We are now seeking an experienced and compassionate Aged Care Coordinator to join our team.
The Position
The Aged Care Coordinator is a part-time position (0.8 FTE) based in our Devonport office, with regular travel to outreach areas across the region. This role is responsible for coordinating and managing aged care services to ensure older Tasmanians receive timely, effective and person-centred support. Balancing administrative coordination with direct client engagement, you will work closely with older people, families, healthcare providers and community partners to develop tailored care plans that enhance wellbeing and independence, particularly in rural and regional communities.
Reporting to the General Manager, you will support integrated service delivery aligned with organisational strategy and aged care sector standards. The role includes assisting clients to navigate systems such as My Aged Care, advocating within complex service environments, monitoring and reviewing care plans, and contributing to continuous service improvement initiatives.
A National Police Check and current Registration to Work with Vulnerable People (RWVP) Clearance are required prior to commencement.
What you'll bring
* You bring experience coordinating or managing care within the aged care or health sector, along with a strong understanding of My Aged Care, the Aged Care Quality Standards and relevant legislation. You are confident supporting clients and families with empathy and clarity, while managing competing priorities in a dynamic environment.
* You have experience in client assessment, care planning and advocacy, and are comfortable working both independently and collaboratively. With strong organisational and problem-solving skills, you are capable of maintaining accurate records, contributing to reporting requirements and supporting equitable access to services across rural communities. Above all, you are committed to person-centred practice and continuous improvement.
The Organisation
The People Project Tasmania is a community-driven, not-for-profit organisation committed to improving health and aged care services across Tasmania. In addition to being one of seven Rural Workforce Agencies funded nationally by the Australian Government, TPPT is also a provider of NDIS Support Coordination, Plan Management and aged care coordination services across the state.
Our main office is located in Launceston, with satellite offices in George Town and Devonport. This role will be based in our Devonport office and will work closely with clients and stakeholders across the Northwest region, with regular regional travel required.
