Job Title: Community Care Specialist
Job Description:
We are seeking a highly skilled Community Care Specialist to join our team. As a Community Care Specialist, you will play a critical role in facilitating the intake, screening, and placement of children and young people with foster and kinship care families.
You will deliver assessments, crisis support, and therapeutic interventions, escalating concerns as needed. Your key responsibility will be to develop and evaluate care plans tailored to individual needs, working closely with community partners to ensure effective support for families.
Your duties will include recruiting, assessing, and supporting foster and kinship carers, facilitating approval processes and providing flexible, in-home support services, including after-hours and on-call duties.
You will work respectfully with Aboriginal and Torres Strait Islander communities to ensure culturally safe and inclusive practices, and lead case management planning and maintain accurate client records.
Required Skills and Qualifications:
* Relevant degree in social work, psychology, or related field – or equivalent experience.
* Strong understanding of child protection, out-of-home care, and relevant legislation.
* Skilled in care assessments and trauma-informed practice.
* Knowledge of challenges faced by Aboriginal and Torres Strait Islander communities.
* Current driver’s license, National Police Check, Working with Children (Blue Card) and Licensed Care Service (Child Protection).
