About the role
Join Human Core Pty Ltd' as a Support Coordinator in our Logan Central office. This is a casual role that involves providing high-quality support to individuals and their families. You will play a crucial part in helping our clients achieve their personal goals and live more independent, fulfilling lives.
What you'll be doing
* Developing and implementing personalised support plans in collaboration with clients and their families
* Coordinating access to a range of community services and resources to meet the individual needs of clients
* Monitoring and reviewing client progress, adjusting support plans as required
* Advocating for clients and liaising with service providers to ensure seamless care
* Maintaining detailed client records and documentation in line with organisational policies
* Participating in regular team meetings and training to continuously improve service delivery
What we're looking for
* Relevant tertiary qualification in community services, social work or a related field
* Demonstrated experience in a similar support coordination or case management role
* Excellent communication and interpersonal skills with the ability to build rapport with diverse clients
* Strong organisational and time management skills to juggle multiple client needs
* Empathy, patience and a genuine commitment to making a positive difference in people's lives
* Willingness to work flexible hours, including some evenings and weekends, to meet client needs
* Current driver's licence and access to a reliable vehicle
