About the Role

The Community Worker/Intake Officer is responsible for centralised management of referrals and intake process for NDIS and Aged Care participants. This includes conducting initial assessments, risk assessments, and completing all required safeguarding to ensure a smooth transition into services.

Responsibilities

* Support participants to make their own choices: Support participants to understand, explore and think creatively about their options, and uphold their decisions.
* Referral & Intake Management: Receive and process referrals for NDIS core services and Aged Care support.
* Conduct initial assessments, including risk assessments, to determine participant needs and suitability for services.
* Gather and verify participant information, ensuring accuracy in documentation.
* Schedule and coordinate intake meetings with participants, families, and support coordinators, internal and external stakeholders.
* Work with relevant staff to complete all necessary paperwork, including consent forms, service agreements and risk assessment documentation.
* Provide clear and transparent information about Far North Community Services' core supports.
* Guide participants and families through the intake process, ensuring they understand their rights, responsibilities and service expectations.
* Address participant concerns, offering person‐centred support and solutions.
* Identify any urgent needs or risks and escalates appropriately.
* Ensure all intake documentation is accurate, complete and compliant with NDIS and Aged Care regulatory requirements.
* Maintain secure and confidential records in accordance with privacy laws and organisational policies.
* Work within the NDIS Code of Conduct and Far North Community Services' policies and procedures.
* Liaise with support coordinators, case managers, Disability Service Manager, Coordinator and allied health professionals to ensure smooth service implementation.
* Build and maintain strong relationships with referrers, community organisations and government agencies.
* Provide updates to management regarding intake trends, service gaps and risk factors.
* Participate in team meetings and contribute to process improvement initiatives.
* Obtain knowledge of and follow Far North Community Services' policies, procedures and processes, using Centro Assist.
* Carry out other tasks designated by the Operations Manager to ensure the organisation complies with NDIS Practice Standards, Aged Care Standards and other relevant legislation, company policy; procedure & process is maintained to high standard.
* Attend and actively contribute to team and organisational meetings, training and organisational events.
* Report any incident or potential for harm (near miss) within all relevant legislation inlcucding WorkSafe WA requirements.

Qualifications & Experience

* The successful applicant will be person centred and participant focused with ability to adapt to different needs and personalities of participants.
* Well developed communication and interpersonal skills including the ability to work and communicate effectively with participants, their families, community services and other professionals.
* Experience in intake, referrals or risk assessment within the NDIS, Aged Care or community services sector.
* Strong understanding of NDIS core supports and eligibility requirements.
* Experience in conducting initial and risk assessments for new participants.
* Excellent verbal and written communication skills, with the ability to engage effectively with diverse participants and stakeholders.
* Strong organisational skills and ability to manage multiple referrals efficiently.
* Proficiency in TRACCS and Microsoft Office Suite.
* Understanding of confidentiality, privacy laws and ethical standards in the disability sector.
* Relevant qualifications in Community Services, Disability, Aged Care or a related field.
* Experience working with culturally and linguistically diverse (CALD) communities and First Nations participants.
* Knowledge of mental health, homelessness and other social factors impacting service access.
