The Role
An opportunity has become available for a Home Care Coordinator to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
- Deliver efficient and effective client focused care coordination of eligible Commonwealth Home Support Program clients
- Undertake effective case planning and management through client consultation, assessment and review utilising the endorsed assessment tools
- Participate in policy and procedure initiatives and continuing quality improvement activities that monitor and improve standards of care
- Manage the performance appraisal and development of staff
About you
**To be successful in this position you will require**:
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella (Chicken Pox), Pertussis (Whooping Cough), Hepatitis B and COVID 19
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required
- A current Aged Care check (QH will facilitate this check)
- Driver Licence
- Declaration of serious discipline history must be disclosed

- Permanent full time
- Base salary ranging from $85 208 - $89 828 per annum
About Us
The Gold Coast Home Care and Allied Health Service is a Commonwealth Home Support Program (CHSP) delivered by Gold Coast Health and funded by the Commonwealth. This community-based program employs approximately 120 staff who deliver in home care to people living across the Gold Coast Health region.
The Home Care and Allied Health service is an entry level service providing a range of programs within the CHSP guidelines including domestic assistance, personal care, social support, group social support, in home respite, and specialised clinical services in Allied Health, including individual and group allied health.
