About the Role
The Community Treatment and Transition Program is a vital component of our mental health services, providing treatment, care, and supervision for individuals transitioning from hospital to community. As a Senior Mental Health Clinician, you will be part of a multidisciplinary team overseeing clients discharged from Thomas Embling Hospital who require linkage support to area mental health services.
You will conduct comprehensive assessments and interventions for high-risk clients in the community, including risk assessments, crisis plans, and care development. Your expertise will ensure seamless transitions between hospital and community settings, promoting recovery-focused programs for consumers with serious mental illnesses.
* Providing high-quality intensive support for treatment-ordered clients post-discharge
* Conducting comprehensive assessments and interventions for high-risk clients
* Preparing case-managed clients for court and engaging in relevant legal processes
Key Responsibilities
Your primary functions will include:
* Case management of forensic clients subject to custodial supervision orders (CSO) granted Extended Leave by the courts
* In-reach support to patients and clinical teams at TEH regarding community transition, including advice and secondary consultations on discharge plans and legal processes
* Participating in service-wide quality improvement activities, including data collection, reporting, and participating in projects and group work
Requirements
To be successful in this role, you will need:
* A minimum of 5 years post-qualification experience in mental health, preferably recent community mental health experience or extensive clinical mental health experience
* Approved degree in Social Work, Occupational Therapy, Psychiatric Nursing, or Psychology, registered with APHRA, or other qualifications approved for membership of the Australian Association of Social Workers
* Demonstrated commitment to mental health consumer recovery, with an understanding of recovery principles and their application
