The successful candidate will provide mental health services in the community, working with consumers experiencing acute mental health problems. Our team provides specialist services for young people, children and families, adults, and older people.

About the Role

The Acute Care Service model of care includes:

* Mental health assessments to consumers within the community experiencing acute mental health problems, and determining an appropriate pathway to care.
">
* Short-term mental health treatment in the community, often as an alternative to hospital admission.
">
* In-reach to hospital to promote effective follow-up post-mental health admission.
">
* Collaborative care for consumers transferring between hospital and community.
">
">
Key Responsibilities

The successful candidate will be responsible for:

* Providing mental health assessments and developing individualized care plans.
">
* Delivering short-term mental health interventions in the community.
">
* Working collaboratively with other stakeholders to ensure seamless transitions of care.

Requirements

To be considered for this role, you will need:

* Recognized nursing tertiary qualifications and current nursing registration with AHPRA.
">
* A current NSW Driver's Licence (P1 and P2 acceptable).

* Demonstrated competencies in contemporary mental health practice.
