Case Manager – Homelessness – Mobile Support

Classification: SCHADS Community Level 4

Full time - Permanent

Location : Manoora – Cairns QLD

Hours : 9am – 5pm ( Monday – Friday )

We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

About the Role

You will provide case management support to individuals experiencing homelessness and risk of homelessness, in person and over the phone.
• Your support will help individuals achieve housing stability, enhance social inclusion, and develop self-mastery—empowering them to sustain tenancies and live their best lives.
• This work is grounded in person-centred case management, drawing on resilience-building, trauma-informed care, child-inclusive approaches, and strengths-based practices.

How You Will Make an Impact
Build strong, supportive relationships with clients that foster self-determination and active participation.

- Build solid, team-oriented relationships within the sector.
- Conduct comprehensive assessments that identify clients’ needs, risks, and vulnerabilities.
- Continuously assess and respond to presenting issues from intake through to case closure.
- Develop individualized case plans in partnership with clients, addressing their goals and the barriers they face.
- Implement case plans with clients’ full engagement and participation.
- Connect clients with appropriate support services and community resources, including advocating on their behalf when needed.
- Regularly monitor, review, and adjust case plans to ensure positive progress and outcomes.

What You Will Bring
• A tertiary qualification in Community Services, Social Work, or a related field (a degree qualification is preferred).
• Relevant experience in the social services sector, particularly in client assessment and case management
• A valid Working with Children Check.
• A current Driver’s Licence
