As a skilled Clinical Nurse Specialist, you will be responsible for delivering complex clinical care in the community.
Key Responsibilities
* Complex Wound Care:
o Conduct thorough wound assessments, including photography and measurements.
o Develop individualized wound management plans.
o Evaluate dressing types and negative pressure wound options.
* Patient Assessment & Care Planning:
o Perform holistic in-home assessments covering physical, psychosocial, and environmental factors.
o Develop person-centered care plans in collaboration with allied health teams.
o Coordinate referrals to relevant health services.
* Clinical Leadership:
o Mentor junior nurses and student placements in the community.
o Lead clinical policies and best-practice guidelines development.
o Participate in clinical audits and quality improvement initiatives.
The ideal candidate will have current AHPRA registration as a Registered Nurse, minimum 3 years of clinical nursing experience, and strong experience in wound care and complex case management.
