Latrobe Regional Health is looking for Residential in Reach Program Care Coordinator to join our growing team on a part time (0.8EFT) ongoing basis.
The Residential in Reach Program Care Coordinator provides advanced clinical assessment, intervention and care coordination for residents living in Residential Aged Care Facilities (RACFs). This role aims to reduce avoidable presentations to the Emergency Department by delivering timely, evidence-based care within the resident's home, supporting facilitated discharge planning, and building the capability of residential care staff to manage increasingly complex clinical needs.

The role of the Residential in Reach Program Care Coordinator is:
* Respond to enquiries and referrals in line with Non-Admitted Patient Activity guidelines and required timeframes.
* Triage and clinical assess RACF residents, providing advanced assessment and interventions as required.
* Develop and coordinated management plans in consultation with General Practitioners, families, and multidisciplinary health professionals.
* Facilitate discharge planning from acute and sub-acute settings to support residents returning safely to residential care.
* Complete accurate and timely clinical documentation in accordance with organisational and legislative requirements.
* Support capacity building within residential aged care facilities through education, advice and collaboration with staff.
About you
Ideal candidates for this role will have:
* Experienced Registered Nurse with advanced clinical assessment and decision-making skills.
* Demonstrates integrity, professionalism and strong communication skills.
* Compassionate, client-focused and respectful when working with residents, families and carers.
* Able to work autonomously while contributing effectively within a multidisciplinary team.
* Adaptable and committed to quality, best practice and continuous improvement.
