**About the Opportunity**
The SWSLHD Residential Transitional Aged Care Program (TACP) is seeking a hardworking, enthusiastic permanent part time Social Worker (Level 1 / 2). This is an exciting opportunity to be part of a dynamic and energetic multidisciplinary TACP team within the SWSLHD. The Residential TACP in SWSLHD operates 12 beds across two aged care facilities.
Residential TACP is a goal-oriented, time-limited, therapy focused program designed to assist aged-care clients to transition back to their home in the community and/or make other long term plans for their care.
You will be responsible for providing appropriate Social Work services change to interventions other than services to TACP clients and their carers. Your clinical caseload will include assessing and providing appropriate assessment, intervention and discharge planning. You will enjoy problem solving and have strong clinical judgement and have a good understanding of the My Aged Care system.
The office for this position is based at Fairfield Community Health Centre (Carramar).
**What You'll be Doing**
Provide Social Work services to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.

**How to Apply**
- A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).
- Demonstrated experience in a health related field and ability to provide a range of Social Work interventions.
- Demonstrated experience prioritising work and managing stressful situations.
- Proven ability to work independently and collaboratively within a multi-disciplinary team.
- Demonstrated high level communication skills (written and verbal) and computer skills.
- Evidence of commitment to clinical supervision and professional learning.
