Your opportunity
The Clinical Nurse Consultant - Chronic Disease Service Coordinator, Charleville, Western Qld, demonstrates accountability at an advanced practice level for managing and coordinating ongoing care for consumers/person with chronic disease within the Primary and Community Care Service and for the provision of specialist nursing advice, consultation, leadership and education to inter-professional team members as relevant to the specific context of the role.
This position is required to travel and work across South West Hospital and Health Services facilities.

Charleville is a designated Remote Area Nursing Incentive Package (RANIP) site in accordance with the Nurses and Midwives (Queensland Health) Award – State 2015. This attracts additional entitlements which may include:
* An annual isolation bonus paid annually
* Return airfares to the nearest provincial city in conjunction with leave
* Additional annual leave per annum
