Job Description:

We are seeking experienced professionals who can make a positive impact by supporting individuals with disability and/or mental health challenges to achieve their goals and live an independent life.

Key Responsibilities:

* Support participants to participate in age-appropriate activities and tasks, including employment, in-home support, and community access.
* Work in accordance with medication administration policies, work health and safety procedures, participant's rights policies, and privacy and confidentiality.
* Monitor the safety and well-being of participants and report to the Client Care Manager.
* Provide one-to-one and two-to-one support in response to individual participants' needs.
* Ensure completion of appropriate documentation, including relevant information on supports provided.
* Work with participants, families, and relevant professionals to support individual goals.
* In conjunction with other staff members, assist in developing, implementing, evaluating training programs to attain identified individual goals (BSP).


Required Skills and Qualifications:

Experience working with people with psychosocial needs or lived experience with supporting moderate to severe mental health illness.

A passion for supporting people to help live their best life.

The ability to see potential in people and show initiative.

Creativity and proactivity in identifying and responding to changes.

Empathy and good listening skills.

Ability to work independently and as part of a team.

Excellent verbal and written communication skills.

Certificate III in Disability or higher.

Driver's License.

First Aid and CPR certificates.

Manual Handling certificate.

NDIS Worker Screening Check SA Working with Children Check.
