The Role
This role supports young people transitioning from custody to community by coordinating post-release care for those with mental health and/or drug and alcohol concerns. As part of a multidisciplinary team, you will build strong partnerships with community services, provide culturally safe and developmentally appropriate interventions, and act as a clinical and cultural resource. You’ll support care planning, deliver brief interventions, and promote engagement with health and support services to ensure continuity of care and positive outcomes for young people and their families.
