Position: Integrated Team Care Coordinator

Position Overview
Provide quality care coordination for referred clients with chronic disease and complex needs to support them in accessing relevant services according to their care plan with the aim of self-management in Primary Health Care (PHC). Work in the team context in undertaking a range of care coordination activities for Integrated Team Care clients with chronic conditions and complex needs in accordance with their management plan.
This position will be based in Toowoomba at our programs office.
Key Duties Include:
Working effectively with Indigenous outreach workers to engage with clients.
Arranging services outlined in the client management plan (e.g. specialist and allied health appointments, arrangements for home help, connections with support groups, etc.)
Ensuring transport arrangements are in place to enable the client to attend appointments.
Involving the client's family / carer as appropriate.
Assisting clients to adhere to treatment programs and develop chronic condition self-management skills.
Assisting the client to participate in regular reviews by primary care providers.
Providing appropriate clinical care consistent with the requirements of the role (this may be in the client's home or clinic setting)
Maintaining quality client medical records
Employ a holistic, culturally appropriate approach to the client, drawing out cultural or social issues which may be impacting the client.
In consultation with other team members, work to develop and implement solutions to client needs (e.g. referral to relevant service providers for support regarding accommodation, finances, well-being, and daily living, etc.)
Use shared understanding of the practical realities facing the client to build a culturally sensitive health plan which increases client ownership and self-determination and communicate effectively.
Advance client access to urgent and essential allied health or specialist services, necessary transport, and GP-approved medical aids through appropriate use of the program funds for eligible ITC clients.
Requirements:
Experience providing one-on-one support toclients.
Ability to collect data,write reports and clientnotes.
Experience working in an Aboriginal community/health/disabilityservice.
Demonstrated understanding of the issues and barriers that impact upon the safety and wellbeing of Aboriginal and Torres Strait Islander people.
The ability to interact and communicate with Aboriginal people from diverse backgrounds, with sensitivity to Aboriginal culture, history, and current social impacts on them.
Effective organisational skills to plan, manage and prioritise tasks and resources to deliver outcomes within time and budget considerations.
Ability to work independently and in a team environment, cope with pressure and able to meet tighttimelines.
The ability to interact and effectively engage with a range of administrative staff, medical practitioners, nurses, and other health professionals and external agencies.
Demonstrated computer skills especially in the use of client information systems orsimilar.
Experience working with external health providers and other community-based services andorganisations.
Experience working with Aboriginal and Torres Strait Islander people and having a clear understanding of Aboriginal and Torres Strait Islander culture.
Tertiary qualification in community services, health, or another relevant professional field (preferred but not essential).
High level of personal resilience and flexibility with the ability to deal with challenging situations.
Demonstrated computer skills especially in the use of client information systems or similar.
