**Job Title:** Home Care Assistant
We are seeking a dedicated and experienced Home Care Assistant to provide high-quality personal care and daily living assistance to our residents. Our ideal candidate will have a passion for delivering person-centred care that promotes independence, wellbeing, and quality of life.
The successful applicant will have the ability to work collaboratively as part of a multidisciplinary team to implement individualised care plans that meet the unique needs of our residents.
**Key Responsibilities:
* Providing personal hygiene, mobility support, and social engagement services to residents
* Collaborating with the care team to develop and implement care plans that promote resident independence and wellbeing
**What We Are Looking For:
* Certificate III or IV in Individual Support (Aged Care) or equivalent qualification
* Minimum 2 years of experience in providing person-centred care to older adults, particularly those living with dementia
