The role
Our Home Health program delivers in-home support services to our customers to help them live independently, supported and empowered, within their own homes and communities. Your clinical environment will be driven by human-centered, holistic, evidence-based care and supported by professional development.
This is a Permanent Part Time opportunity of 30hrs, supporting our customers and team in the Yamba, Maclean and Iluka area.
Tasks may include but not limited to:
* Delivering individualised clinical based nursing care to customers living in the home environment such as wound care, catheter care, PEG changes and care, bowel care and medication management
* Conducting nursing assessments and developing care / support plans
* Supporting, mentoring and delivering clinical training to our Care Workers in the community
* Providing clinical nursing advice and support to our customers and their families
About you
Above all, you will align to our Values of Bold, Warm and Honest. You will be able to demonstrate empathy and the ability to provide excellent care for your customers.
* Hold a Current APHRA registration and be able to provide a copy of your nursing degree
* Current First Aid Certificate and Right to Work (provide evidence if applicable)
* Current Australian driver licence (full and no restrictions) and own a fully maintained, registered and insured vehicle
* Willingness to travel - role does involve driving and travel between customers and areas as required
* Experience in a community or similar setting would be highly regarded
* Demonstrated knowledge or willingness to learn aspects of catheter care, catheter changes, wound care, PEG management, chronic & complex care and writing care plans
