NDIS Support Coordinator

Role Responsibilities:
•Coordinate formal and informal supports
•Manage clients with complex needs and handle crisis situations
•Collaborate with a Multidisciplinary Panel
•Engage with Mental Health, Justice System, Housing, and Health sectors
•Undertake clerical administration and report writing
•Network and communicate with key stakeholders
•Work independently
•Work in a sensitive, complex and demanding environment with a high degree of autonomy and self-management.
Qualifications & Experience:
•Minimum of Certificate IV in Community Services or related discipline
•Minimum 2 years' experience working with participants with moderate to complex mental health issues, high medical needs, and/or behavioural support needs
•Experience in Forensic Disability or Forensic Mental Health is a plus
•Demonstrated in-depth knowledge of the NDIS and experience working with its participants
•Valid Driver's License with a suitable, insured vehicle
•A current Working with Vulnerable People registration
•Recent police check (or willingness to obtain one)
