Job Overview

Clinical Aged Care Assessors play a pivotal role in ensuring that older adults receive the care and services they require. This position offers flexibility and autonomy, with opportunities to work across metropolitan and regional locations.


Key Responsibilities

* Collaborate with clients, families, and multidisciplinary teams as needed
* Contribute to promoting a supportive and positive team culture
* Accurately assess client needs using the Integrated Assessment Tool (IAT) to determine eligibility for home support services
* Review and update support plans to meet changing client needs
* Work autonomously and as part of a broader team to achieve key performance indicators within specified timeframes
* Maintain currency of knowledge on funded and non-funded aged care services
* Participate in continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes


Required Qualifications and Skills

* Tertiary qualification in a health-related discipline related to health, aged care, or a specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist, or Social Worker)
* Current unrestricted registration with the Australian Health Practitioner Regulation Agency (AHPRA)
* Australian driver's license and access to a registered, insured vehicle
* Minimum one year experience delivering services in aged care settings and/or to older adults, including those living with dementia
* Willingness to obtain a Police Check
