Job Overview
We are seeking a skilled professional to oversee the implementation and coordination of our Integrated Chronic Disease Care program in the South West region.
This role will provide strategic leadership, collaborating with stakeholders to optimize resource utilization and ensure service delivery complements existing primary care services.
Key Responsibilities:
1. Develop and implement a collaborative approach to chronic condition care, ensuring optimal use of resources.
2. Collaborate with community stakeholders to enhance existing primary care services.
3. Support GP-led/informed care utilizing team-based care principles.
4. Coordinate care coordination to support client self-management.
5. Ensure consumer-centered care through shared decision-making partnerships between clients, families, carers, and referring GPs.
6. Lead an existing nursing and allied health team based throughout the South West region.
7. Work collaboratively with other Oseca services and program managers to provide coordinated services.
8. Manage report preparation, data collection, and program evaluation.
9. Review, develop, and implement guidelines, policies, and procedures for the program as required.
Requirements:
1. Registered Nurse with a background in clinical nursing within Aged Care Practice Nursing or similar environment.
2. Experience in supporting clients with Diabetes.
3. Strong leadership skills and passion for clinical excellence.
4. Experience in managing teams and optimizing clinical outcomes within budget constraints.
5. Minimum degree qualifications as a Registered Nurse.
6. Relevant postgraduate qualifications strongly preferred.
7. At least 5 years experience in a Clinical Nurse role.
8. Demonstrated people leadership experience.
9. Proven experience in chronic condition management.
10. An understanding of innovative care models, integrated care, or partnership-based service delivery.
11. Current Registration with AHPRA.
12. The right to work in Australia and be based in the South West currently.
13. Pre-employment Health Declaration.
14. A valid WA drivers license.
15. A valid and clear police check.
16. A Working with Children Check WA (WWC).
