Overview
Join the Hunter Valley Transitional Aged Care Program (TACP) and make a real difference in the lives of older people as they regain their independence after a hospital stay. Our multi-disciplinary team of Allied Health and Nursing professionals work closely with clients and their families, focusing on enablement and providing tailored, slow-stream therapy that helps people improve their function, confidence, and safety at home.
The service runs year-round, Monday to Friday, 8:00am–4:30pm, offering variety and stability for our Allied Health staff.
Responsibilities
- Deliver high-quality Social Work services within the Hunter Valley Transitional Aged Care Program.
- Work with a dedicated team of Allied Health and Nursing professionals to support patients and families through slow-stream therapy, aiming to restore function, boost independence, and ensure safety at home.
- Conduct home visits as part of a service operating daily from 8:00 am to 4:30 pm, 365 days a year.
- Participate in comprehensive onsite training (including CHIME) and ongoing professional development with orientation, regular clinical supervision, and strong peer support.
About You / Qualifications
We’re looking for a dedicated and compassionate Social Worker who is committed to making a real difference in the lives of patients and their families or carers. You’ll share our passion for improving outcomes, helping patients maintain their quality of life, and supporting them to achieve their functional goals.
You’ll bring self-motivation, autonomy, strong time management, empathy, enthusiasm, flexibility, teamwork, and the ability to work independently. Excellent verbal and written communication skills and a commitment to safe, compassionate, person-centred care are essential.
Requirements: Must hold at minimum a bachelor degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.
Remuneration and Conditions
Remuneration: $112,396 - $116,038 per annum + super + salary packaging. Employment Type: Permanent Full Time. Position Classification: Social Worker Level 3.
