Overview
Clinical Aged Care Assessor -Mid North Coast role at Catholic Healthcare. ACAS positions offer flexibility and autonomy and are based across metropolitan and regional locations, with most roles involving local travel within each region.
Responsibilities
- Collaborating with clients, families and multidisciplinary teams as required
- Working with colleagues to promote a supportive and positive team culture
- Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services
- Reviewing support plans to meet client’s changing support needs
- Working autonomously and as part of a broader team to deliver against key performance indicators within identified timeframes
- Maintaining currency of knowledge relating to funded and non-funded aged care services
- Contributing to continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes
What you’ll need
- Tertiary Qualification in a health-related discipline related to Health, Aged Care or a related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker)
- Current unrestricted registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Australian driver’s license and access to a registered, insured vehicle
- Minimum of one year experience delivering services in Aged Care settings and/or to older persons, including people living with dementia
- Willingness to apply for a Police Check

- Flexible hours and days – Monday to Friday, part time 3 days per week; no shift work
- Pay rate – $54.47 per hour + superannuation
